Advance Care Planning Initiative Request for Proposals

This announcement solicits applications for Community Outreach Partners in support of the South Carolina Coalition for the Care of the Seriously Ill Advance Care Planning Initiative. The ACP Initiative is supported by The Carolinas Center through a grant from the BlueCross BlueShield of South Carolina Foundation. The purpose of this grant is to improve the quality of and accessibility to ACP conversations through shared decision making for all South Carolinians. The intent of this initiative is to ensure these quality conversations will be reflected in documents that are understandable, accessible and honored. Outreach efforts will be targeted to communities characterized by predominantly low income, underserved, uninsured and/or rural populations.
Through the generous support of the BlueCross BlueShield of South Carolina Foundation, The Carolinas Center in partnership with SC CSI will provide approximately $600,000 to assist Community Outreach Partners in the implementation of activities to improve ACP and to promote the use of ACP documentation for a period of two years.
Background: In 2016, approximately 1 percent of Medicare beneficiaries in South Carolina engaged in a documented ACP billed discussion according to the Centers for Medicare & Medicaid Services Chronic Condition Data Warehouse ( Source). Despite increased awareness of the importance of ACP, these discussion are not occurring on a regular basis. In an effort to address this issue in our community, the ACP Initiative’s goals are to increase awareness of and the quality of ACP conversations, improve the health care professional’s ability to initiate and integrate ACP conversations and processes into their daily practice. In addition, the ACP Initiative seeks to improve the accessibility to ACP documents through technology.
Scope of Work: Grant funding will support Community Outreach Partners through collaboration, adoption of evidence-based or promising practice models, demonstration of positive outcomes, replicability and sustainability. Community Outreach Partners will receive education and training, financial support up to $100,000, consultation regarding ACP issues and on-going technical assistance related to implementation.
